If National Math and Science Initiative notified you of a
Data Incident in or around February 2022, you are eligible
to enroll at no cost in financial protection services. 

Enrollment is now openfor Financial Shield Total. You will receive one year of Financial Shield Total service from the date of your enrollment. To enroll in Financial Shield Total, visit the website app.financialshield.com/enrollment/activate/nati and enter your Activation Code which was mailed to you in the Class Notice. Please note that this code is unique to you and should not be shared with anyone else for enrollment. In addition to your activation code, you will need to provide your name, address, date of birth, telephone, a valid email, and your Social Security number to complete your enrollment. The provision of this information is necessary to provide you with the full benefits of Financial Shield Total.  The deadline to enroll is October 1, 2024.  After October 1, 2024, the enrollment process will close, and your Financial Shield Total code will no longer be active.

YOUR  LEGAL RIGHTS AND OPTIONS IN THIS SETTLEMENT

EXCLUDE YOURSELF FROM THE SETTLEMENT

You will receive no Credit Monitoring Services, but you will retain any rights you currently have with respect to Defendant and the issues in this case. The deadline to exclude yourself from the Settlement is May 6, 2024.

OBJECT TO THE SETTLEMENT

Get no benefits from the settlement. Keep the right to bring a separate lawsuit about the same issue at your own expense.

ATTEND THE FINAL APPROVAL HEARING You or your attorney may attend and speak about your objection at the Final Approval Hearing. The Final Approval Hearing was held on June 12, 2024.  This case was approved at the Final Approval Hearing.

DO NOTHING Write to the Court about why you don’t like the settlement. If the settlement is approved, you will still automatically receive a check or account credit and give up the right to bring a separate lawsuit about the same issue.
